Donation terms and conditions:

When donating medication, you agree to adhere to the prinicple of maintaining the safety of the supply chain:

• Refrigerated items will be shipped with proper cold packing

• All items submitted will have the accurate lot #, expiration date, and NDC # completed on the shipping manifest.

• Unacceptable shipments will be returned or the donor will agree to pay the cost of destruction.

• Hazardous materials will be properly packaged within a separte bag, in a manner to avoid breakage, and be marked as Hazardous on the outside of the package.

• The medications donated have been in your organization's control since they arrived.

• They are unadulterated and safe for use by humans to the best of your knowledge.

• Have been obtained through a source known to be compliant with DSCSA requirements.
